Client Guidelines

Eligibility:  Client must be age 60 or older, and reside within
the Village of Groveport or the unincorporated areas of
Madison Township. Mentally or physically challenged clients under age 60 are eligible provided a physician's certification of disability is submitted at time of application. Any client requiring assistance of any kind must be accompanied by a caregiver/companion.

Service Area:  Medical appointments will be accepted for physicians’ offices within Franklin County. If feasible, clients will be encouraged to select physicians or therapy services within the general area. Transport for personal appointments will be accepted within a 10 mile radius of the Village.  The boundaries for personal trips are Broad Street to the North, High Street to the West and the county lines to the East and South. The service is offered door to door at no cost to the client. Priority will be given to medical appointments.

The following guidelines apply to all clients.

1. Clients must walk/wheel to and from the van and not require the assistance of driver. If assistance of any kind is required, the client will arrange for a caregiver age 18 or over to accompany the client. The caregiver will assume responsibility for assisting the client to and from the van, up and down steps, shopping, opening doors, etc.  Caregivers will be required to sign this form.

2. All clients will be safely secured at all times with seat belts while transported and, if necessary, floor-mounted wheelchair straps.

3. Combative or seriously ill residents cannot be safely transported and must make alternate arrangements.

4. The Senior Transportation Program has been developed to provide assistance for scheduled trips and is not equipped to handle emergency calls. In an emergency situation, 911 or a private ambulance  firm are alternative emergency transportation.

5. Any relevant medical condition must be communicated to the drivers or schedulers (e.g. anxiety, seizures, pain, confusion.)

6. A pre-screening home visit will be held prior to acceptance in the program. The visit will identify any significant barriers or limitations to safe transportation, as well as answer any questions the client may have.  A photograph of each client will be taken at that time.

7. Wheelchairs must be of standard size. Oversized wheelchairs may be unable to safely fit onto the wheelchair lift. Motorized 3-wheel scooters are not permitted.

8. Staff are unable to make additional unscheduled stops, e.g. “also run into the grocery for just a couple things.” Our drivers have tight schedules and your unscheduled stop may make another client late for a doctor’s appointment.

9. Social outings (volunteerism, entertainment, etc.) will only be accommodated if the schedule permits and only for locations within the general area.

10. Grocery shopping and other trips that are not dependent upon a certain time will need to be flexible.

11. Non-medical standing appointments are not accepted. Please call the office each time you would like transport.

12. The Senior Transportation Program reserves the right to limit service for non- medical appointments to accommodate the number of clients. No client may utilize the service more than two times per week.

13. Transportation for appointments will be provided between 8:00 am - 1:00 pm, with a minimum of two days’ notice. 

Should conditions exist which negatively impact client or driver safety, the Groveport- Madison Township Senior Transportation Program reserves the right to deny service.
